Ces quelques points concernent la matériovigilance :
Enkele punten met betrekking tot materiovigilantie:

 Mise en place de point de contact dans les hôpitaux
 Etablissement de procédures afin de faciliter la notification de la part des points de
contact matériovigilance
 Plus de publications vers l’extérieur
 Sensibilisation accrue des acteurs vis à vis de la matériovigilance
 Optimalisation de la prise en charge des dossiers au sein de l’afmps

 Oprichten van een contactpunt in de ziekenhuizen
 Opstellen van procedures voor het vergemakkelijken van de notificatie van deze
contactpunten voor materiovigilantie
 Meer publicaties naar buiten toe
 Sensibiliseren van de actoren m.b.t. materiovigilantie
 Optimaliseren van het dossierbeheer binnen het FAGG

Pour ce faire, 2 groupes de travail ont été mis en place, une demande en personnel supplémentaire
réalisée et une modification de la législation est en cours.

Om dit te realiseren werden er twee werkgroepen georganiseerd, extra personeel gevraagd en een
wijziging aan de wetgeving is lopende.

Il y a lieu de savoir qu’à l’heure actuelle le plan n’est pas encore approuvé au niveau du conseil des
ministres.

Het is noodzakelijk om te weten dat, op dit moment, het plan nog niet goedgekeurd werd door de Raad
van Ministers.

Recent Crash of TACSI test machine (Project: whole blood separation)
Dear Wilfried,
Hereby, we wou1d 1ike to inform you about the resu1ts of our fist investigation on above
mentioned matter:
1. Without the broken device and parts a fina1 statement about the reasons for the crash
cannot be given. Therefore we kind1y ask you to return:
a. The broken T ACSI device
b. The broken parts assemb1ed in bags as follows:
i. from inside the device,
ii. from outside the device, and
11. the parts that stuck in the wall
2. We consider the risk for the devices present in the field to be acceptab1e as long as they
are working with 1ess than 1.500 RPM. This concerns the use ofthe standard version of
the TERUMOCS T ACSFM Kit as well as the use ofthe new TERUMOCS T ACSFM Kit with
ready to use cassette. However, we intend to take the following immediate actions:
a. Software-update: limitation ofspeed to 1.500 RPM
b. Modification of centrifuge lids
c. Update ofUser Manual with respect to speed limitation
In addition we will again check the durability ofthe system boxes (current design) and - if
necessary - limit the product life expectancy.

3. We are p1anning areplacement action as follows:
a. Shipment of rep1acement kit (27 lids without handle, software update, and eventually
other items as well) inc1uding full description to Terumo Europe. The rep1acement kits
wil be supp1ied free of charge.
b. Exchange ofmechanica1 parts and software update by Terumo Europe service staffat
all sites according to the description provided by Rettich.
c. Documentation of above mentioned exchange and software update and report to
Rettich afterwards.
d. Collection of old lids without hand1es and their return to Rettich factory.
We expect to have the rep1acement kits availab1e in 5-6 weeks (November 2009).
4. In regards to the feasibi1ity study for the who1e b100d separation with 2.200 RPM we ask
you to stop all activities. We have already redesigned the system boxes to where they
withstand higher g- forces and we shall test them internally before re1easing them for
further testing.
