however flimsy or well-founded, and having the empirical basis that permits it
to survive the demise of its rationale. As to treatment, externalism directs
etiology to the very cause of the illness, generalism invites sparse and at most
symptomatic treatment, letting nature do the real job of healing. We tend to
side here with generalism, though, of course, more in skepticism than in
affirmation.
II. PSYCHIATRY:
1. Theory:
Psychiatric theory always was very conspicuously extremely externalist,
yet it hardly ever was either medical or scientific: it usually was metaphysical, at
times under the guise of commonsense - including the commonly accepted
ideas of the society in which it was advocated, magic hardly ever excluded - at
times under the guise of science - and so pseudo-scientific par exellence. Often
one author exhibited both generalist and externalist traits, and then in
extremist fashions. One author could also simultaneously employ both
pseudo-science and metaphysics. This was conspicuously so with Pavlov and
more so with Freud.
The only medically proper part of psychiatric nosology is the Freudian
idea that the etiology of all psychic ills is that of an improper self-cure of
anxiety, akin to a cutaneously healed yet subcutaneously still infected wound.
Yet this defective cure is stable due to ambivalence, as Freud also noted. The
idea that the object of ambivalence is exclusively sex, or anything else, is silly: it
is never exclusive unless, perhaps, it is the patient's autonomy - in the
abstract.
2. Practice:
No traditional psychiatric diagnosis is at all up to par except for the
Freudian diagnosis of conversion hysteria and of clear-cut phobias (including
hysterical blindness, and, by extension, la belle indifference). In our Paranoia
we have offered tools for diagnosing psychoses; in the present study we have
slightly modified our proposal so as to add etiology to the picture. The etiology
