Overview of Failure Modes and Effects Analysis
Whereasan RCMAnalysisevaluatesasystem with respectto its functionfor thepurpose
of determininghowto avoid functional failures, aFailureModesand EffectsAnalysis
(FMEA)Analysisfocuseson theequipmentand locationsthemselvesand analyzeshow
each pieceof equipmentor locationcan fail and theeffectof each failure. In other
words, an RCMAnalysisis conducted for thepurposeof maintainingthefunctionof a
system. An FMEAAnalysisis conducted for thepurposeof maintainingthesystem itself.
Specifically, FMEAis amethod of identifyingthepotential failuresof equipmentand loc-
ations, describingthepossibleeffectof each failure, and makingrecommendationsfor
actionsthatcan betaken to preventthefailuresfrom occurring. In Meridium APM, an
FMEAAnalysisconsistsof thefollowingcomponents:
Asset: After you createtheroot-level FMEAAnalysisrecord, which servesas the

baserecord to which all analysisnodesarelinked, you can defineequipmentand
locations, which definewhatis beinganalyzed bytheFMEAAnalysis. Each FMEA
Analysiscan haveoneor morepiecesof equipmentor locations, which arestored
as RCMFMEAAssetrecords.
An FMEAAnalysiscan contain"virtual" equipmentand locations, which areRCM
FMEAAssetrecordsthatarenotlinked to Equipmentor Functional Location
records, and actual equipmentand locations, which areRCMFMEAAssetrecords
thatarelinked to Equipmentor Functional Locationrecords.
Failure Modes: After you havedefined theequipmentand locationsfor an FMEA

Analysis, you can defineoneor morefailuremodesfor each pieceof equipment
and location. Failuremodesdescribethewaysin which each pieceof equipment
and locationfailureis likelyto occur and arestored in RCMFMEAFailureMode
records.
Failure Effects: For each failuremode, you can then definefailureeffects, which

describetheconsequencesof afailureoccurring. Failureeffectsmightdescribe
thesafety, environmental, and economic (or production)impactassociated with a
failure. In Meridium APM, you can defineaRisk Matrix thatwill beavailablein the
analysisand can help you definetheeffectsof each failuremode.
Recommendations: For each failureeffect, oneor morerecommended actions

can bedefined and stored in an RCMFMEARecommendationrecord. ARecom-
mendationrecord defineswhatshould bedoneto avoid therisk defined bythe
FailureModesand failureeffects. Recommendationscan bedetermined, in part,
usingtheDecisionTreedocumentthathasbeen setup for usewith FMEAAna-
lyses.
In addition, for each FMEAAnalysis, you can definean FMEAteam, which is alistof
peoplewho areinvolved in theanalysis.
