Foreword
The incidence of invasive fungal sinusitis has seen a steep rise in recent years. It is
commonly seen in immunocompromised patients. However, more and more cases
are being seen in patients without obvious immunodefi ciency, which is an alarming
trend. Allergic fungal sinusitis is an entity that the nasal endoscopist encounters
frequently while dealing with sinonasal polyposis, and surgery remains the main-
stay of management. There is, however, divided opinion on whether these patients
merit antifungal treatment. Invasive fungal sinusitis, on the other hand, is treated
with antifungal drugs complemented by aggressive surgical debridement. Dr. Gauri
Mankekar, who is a general otolaryngologist, has for several years focused her
attention on the problem of invasive fungal sinusitis and has therefore gained
immense experience in managing these patients both surgically and medically.
This book is a result of this experience and deals with the subject in a very compre-
hensive manner. Starting with a very systematic classifi cation, the book gives a
very clear picture of the variety of presentations and diagnosis of fungal sinusitis.

The chapters on microbiology, pathology and radiology have been dealt with by
experts in their respective fi elds in a very succinct manner. The medical manage-
ment has been exhaustively covered so that the reader has complete information on
the subject. The inclusion of “case studies” has been a very relevant and informa-
tive addition. This book will be of great value and is a “must read” for both the
postgraduate student and the practicing clinician. Dr. Gauri Mankekar deserves to
be complimented on her dedicated effort in bringing out this book.

Dr. Gauri Mankekar
Several years ago I encountered my fi rst paediatric patient of mucormycosis. The
child had juvenile diabetes with ketoacidosis. The surgical treatment paradigm then
was “aggressive debridement”. It was indeed challenging to be “aggressive” with this
small child, and I followed a “conservative approach” – restricting to repeated endo-
scopic debridement without orbital exenteration or palatal removal. Like most clini-
cians in such situations, there was the dilemma, then, whether “conservative” was the
best approach. Luckily the child responded to treatment and I got interested in the
management of invasive fungal sinusitis, but there wasn’t a single comprehensive
book on the subject – only several hundred published papers. This led to the idea of
writing this book.
This book attempts to provide comprehensive information on invasive fungal
sinusitis, the controversies regarding classifi cation as well as its management. My
colleagues from microbiology, pathology and radiology have provided an insight

into the diagnostic aspects of the disease, while Dr. Soman, the infectious disease
specialist, has provided a succinct chapter on medical management which forms the
mainstay of the treatment. The chapter on case studies was included to sensitize
clinicians to the clinical scenarios which could lead to confusion in diagnosis with
subsequent delay in management.
I hope this book will benefi t clinicians in managing patients with invasive fungal
rhinosinusitis.
