Acknowledgements
The Erie St. Clair Local Health Integration Network (LHIN) team thanks everyone who helped develop
this Integrated Health Service Plan. We are grateful to those who attended and participated in the
various engagement mechanisms, sessions, presentations, meetings and surveys throughout our
region over the past several months. A special thanks goes to those people who took the time to serve
on committees (and similar structures), along with colleagues within the Ministry of Health and
Long-Term Care and the staff of other LHINs for sharing their perspectives with us. We are pleased to
forward a plan which provides an overview of health services, needs, actions and targets within our
LHIN. It is our intention that this plan captures and expresses the best information and solutions to
enhanced service delivery available to us. While the scope of the plan encompasses a region where
over 649,000 people live and outlines multiple local strategic priorities for change in the future, the
intention of this plan is simple: to help improve the health care experience for the citizens of the Erie
St. Clair LHIN.
