MAMMARY IMPLANTS SILICONE GEL-FILLED

DESCRIPTION
Gel-filled mammary implants are designed for use in cosmetic augmentation mammaplasty or reconstruction of the breast and are of round or anatomical shapes. The silicone elastomer shell may be
produced smooth or with the outer surface textured to offer a disruptive contact surface for collagen interface and filled with a cohesive or high cohesive gel.

INDICATIONS
Eurosilicone gel-filled mammary implants are indicated for the following:
• Cosmetic Augmentation Surgery
• Augmentation and contour correction of congenital anomalies of the breast
• Reconstruction of the breast following subcutaneous mastectomy and other suitable mastectomy procedures or trauma
• Combined breast and chest wall abnormalities
• Replacement of implants for medical and cosmetic reasons

CONTRA-INDICATIONS
The use of these implants is contraindicated in patients who have one or more of the following conditions:
• FIBROCYSTIC disease
• INSUFFICIENT TISSUE covering due to radiation damage on the chest wall, tight thoracic skin grafts or radical resection of the pectoralis major muscle
• EXISTING LOCAL OR METASTATIC CARCINOMA of the breast
• Suppressed/compromised immune system
• A history of sensitivity to foreign materials or atopy or previous UNSATISFACTORY HISTORY of augmentation/reconstruction
• PHYSIOLOGICALLY/PSYCHOLOGICALLY unsuitable patient
• ACTIVE INFECTION anywhere in the body
• Recent history of breast abscess
• A history of compromised wound healing or regional swelling
• Any other serious medical condition
• These devices are not intended for the treatment of children or pregnant or nursing women

IMPORTANT: The surgeon is responsible for conducting the medical assessments regarding the patient’s eligibility for implantation and for deciding on the most suitable surgical technique for both the
patient and the type of implant chosen.
Breast implants are known to give patients psychological satisfaction. However, as it is the case with any type of medical procedure, there are risks and complications that may arise. Before surgery, the
surgeon must provide the patient with all the information contained in this instructions leaflet, ensure that the patient understands all the terms and get his/her informed consent. The surgeon must inform
the patient of the risk-benefit ratio associated with this procedure. All possible complications and warnings must be discussed prior to surgery. The patient must be informed of the fact that implants have a
limited lifetime and that inserting an implant may require removal or replacement, which could entail another surgical procedure.
Clinical monitoring of Eurosilicone implants has enabled, to date, to establish implant durability of 82.4% after 10 years (risk of a first reoperation = 17.6%, using the Kaplan Meier method).
During a mammography, the radiologist must be informed that there is an implant in the breast in order to adapt the mammography pressure accordingly.
The surgeon or doctor must be notified about the implant in the event of any surgical intervention in the breast area.
A doctor or pharmacist must be consulted before applying any topical medicines (such as steroids) in the breast area.
The presence of breast implants could delay the detection of breast cancer by self-exam. For this reason, patients must be informed that they need to consult a surgeon and/or doctor for appropriate
medical monitoring, as well as regular breast cancer screening. Patients must also be informed that they need to consult a doctor if they suspect complications particularly in the event of trauma or
compression caused, for example, by violently massaging the breasts, a sporting activity or wearing seat belts.
After surgery, the surgeon will provide the patient with the device’s identification card which the patient must keep on their person at all times in order to facilitate emergency medical care in particular.
N.B.: Although Eurosilicone implants have not been specifically tested for use in an MRI, please note that Eurosilicone’s Gel-filled Mammary Implants are all manufactured from medical
implant grade silicone materials which are compatible with MRI Scan. The implantable silicone grade material is the same as that used in other Euroslicone silicone implants where patients
have undergone MRI Scans and no compatibility issues have been reported to date.

POSSIBLE COMPLICATIONS AND WARNINGS
Possible complications known to be associated with gel-filled mammary implants are as follows:

Other potential complications associated with all surgical procedures should be discussed with the patient. These include but are not limited to: infection (see below); haematoma (see below); serous fluid
accumulation (see below); loss of sensation (see below); reaction to medication; nerve damage; patient intolerance to any foreign implant; and poor wound healing.

CAPSULAR CONTRACTURE
Fibrous capsular contracture is a common complication following breast implant surgery. While formation of a fibrous tissue capsule surrounding the implant is a normal physiological response to a foreign
body, not all capsules contract. Contracture of the fibrous capsular tissue surrounding the implant may result in firmness, discomfort or pain in the breast, distortion of the breast, palpability of the implant, or
displacement of the implant. The etiology of capsular contracture is unknown, but is most likely multifactorial. Contracture develops to varying degrees, unilaterally or bilaterally, and may occur within
weeks to years after surgery. Severe cases may require surgical intervention. Capsular contracture may recur after capsulotomy or capsulectomy. Lumps perceived as capsular scarring have delayed
the diagnosis of palpable tumour. Questionable lumps must be promptly investigated.

CALCIUM DEPOSITS
Calcification commonly occurs in mature breast tissue with or without implantation. Calcification is also known to occur after implantation of a foreign body, although the etiology is unknown, and reported
cases are rare. Microcalcification after implantation typically occurs on or around the fibrous capsule in thin plaques or accumulations. Extensive microcalcification may cause breast hardness and
discomfort, and may necessitate surgical intervention.

EXTERNAL CAPSULOTOMY
Treatment of capsular contracture by external manual compression may cause the shell to weaken or rupture. There have been reports of rupture and Eurosilicone recommends against the external

capsulotomy procedure and is not responsible for the integrity of the implant should manual compression or other external stress techniques be used.

RUPTURE OF GEL IMPLANTS
Gel rupture can occur post-operatively from damage to the implant during handling or surgery. Rupture of the shell can also occur from contracture, trauma or excessive manipulation. Ruptures of
unknown etiology have also been reported. Because of the cohesiveness of the gel material, some gel ruptures can go undetected unless surgery occurs for another reason (e.g. size exchange). Despite
the cohesive properties of the gel, extravasation out of the surgical pocket can occur under pressure, in which case additional surgery may be required to retrieve the gel. Inflammation and formation of
silicone granulomas have been reported. If shell rupture is suspected, the implant should be removed.

INFECTION
Pre-existing infection not resolved before implant placement increases the risk of periprosthetic infection. Do not expose the implant or filling accessories to contaminants, which increases the risk of infection.

Infection is an inherent risk following any type of invasive surgery. Infection around a breast implant may occur within days, weeks, or even years, after surgery. Signs of acute infection reported in associa-
tion with breast implants include erythema, tenderness, fluid accumulation, pain and fever. Signs of subclinical infection may be difficult to detect. Postoperative infections should be treated aggressively
according to standard medical practices to avoid more serious complications. Infection that is unresponsive to treatment or necrotizing infection may require implant removal. Capsular contracture may
be related to infection in the area surrounding the implant.

GRANULOMA
Granuloma formation is a common tissue response to the presence of foreign materials. It Is possible for a tissue reaction to be caused by the presence of the implant leading to formation of silicone
granuloma.

HAEMATOMA/SEROMA
Haematoma and serous fluid accumulation are complications associated with any type of invasive surgery. Postoperative haematoma and seroma may contribute to infection and/or capsular contracture.
Postoperative haematoma and seroma may be minimised by meticulous attention to haemostasis during surgery, and also possibly by postoperative use of a closed drainage system. Persistent, exces-
sive bleeding must be controlled before the device is implanted. Any postoperative evacuation of haematoma or seroma must be conducted with care to avoid contamination or damage to the implant.

EXTRUSION
Unstable or compromised tissue covering and/or interruption of wound healing may result in exposure and extrusion of the implant. Causes or contributing factors may include infection, wound dehis-
cence, necrosis with or without infection, capsular contracture, closed capsulotomy, unsuitable skin flap, improper size and placement of implant, and/or tissue erosion associated with implant folds.
The incidence of extrusion has been shown to increase when the prosthesis has been placed in injured areas: scarred, heavily irradiated or burned tissue or crushed bone areas; where severe surgical
reduction of the area has been performed; and where steroids are used in the surgical pocket.

WRINKLING AND FOLDS
Thin or inadequate overlying tissue, patients with little or no subcutaneous fat, implants which are too large for the surgical pocket or the anatomical structure of the patient, and subcutaneous placement
may contribute to palpable, or visible, wrinkles and folds. Folds may result in thinning and erosion of adjacent tissue and erosion of the implant. Signs of skin inflammation, such as tenderness and ery-
thema, may indicate thinning or erosion and must be promptly investigated. Palpable wrinkling / and or folds may be confused with palpable tumour, and questionable cases must be promptly investigated.

SENSATION
The risk of temporary or permanent dysesthesia exists following any invasive surgical procedure. Careful surgical technique can minimise, but not preclude, this risk. Dysesthesia of the nipple/areolar complex,
and less frequently, the breast area in general, has been reported following implantation and may be temporary or permanent. The risk of neurological impairment increases with more extensive surgery.
Surgical implantation of a mammary prosthesis may interfere with the ability to breast feed. However, it should be noted that previous breast reconstruction surgery, may be the initial cause of this
interference.

INTERFERENCE WITH MAMMOGRAPHY
Standard positioning techniques have shown significant limitations when used for imaging augmented breasts. The implant can interfere with the detection of early breast cancer through mammography
by obscuring some underlying breast tissue and/or by compressing overlying tissue which can ‘hide’ suspicious lesions in the breast. Patients should be instructed to request radiologists who are expe-
rienced with the most current radiological techniques and equipment for imaging breasts with implants, and to inform their radiologists of the presence, type, and placement of implants and if anatomical
implants, they should be aware of orientation marks on the anatomical implant.

DISSATISFACTION WITH RESULTS
The complications of incorrect size, misplaced scar location, hypertrophic scarring and those listed below are usually related to surgical technique. Careful size selection, creation of an appropriate and
adequate size surgical pocket, and use of current accepted surgical procedures are the surgeons responsibility.

DISPLACEMENT OF IMPLANT
Implants may displace with accompanying discomfort and/or distortion in breast shape. Difficult placement techniques may increase the risk of displacement by reducing pocket size and placement
accuracy. Displacement may require surgical intervention.

ASYMMETRY
Asymmetry may result from improper initial placement, displacement, or failure to correct pre existing asymmetry through variation in individual implant size. Pre-existing asymmetry may not be entirely
correctable. Asymmetry may also be a symptom of capsular contracture, fluid accumulation, infection, postoperative breast dysplasia, unilateral discrepancy in muscle development, or deflation, requir-
ing further investigation.

USE OF DRUGS
Eurosilicone can neither predict, nor warrant, the safety of intralumenal introduction of any drugs, including, but not limited to, anaesthetic, steroid, antibiotic and vitamin solutions. If such use is contem-
plated, the appropriate drug manufacturer should be consulted.

EXPLANTATlON OF MAMMARY IMPLANTS
Although there are no definite reports in the medical literature, it is anticipated that a large number of mammary implants will be surgically explanted or replaced. This is not unlike many other plastic
surgery procedures which are commonly repeated to maintain patient satisfaction. Due to the wide variety of patients’ physical responses to breast implant surgery, differences in surgical techniques and
medical treatments, as well as potential complications, patients should be advised that these should not be considered lifetime implants and explant surgery may be indicated at any time. Eurosilicone
makes no representations for the term of implantation of the device.

SILICONE ALLERGY
Although silicone allergy is extremely rare, this risk cannot be completely ruled out. 904224 v.06 –10/2017 | 3

BREAST FEEDING
The implantation may affect the milk production during nursing, particularly, peri-areolar incision may reduce considerably the possibility of breast feeding.

PAIN
Any unexplainable pain, not associated with any of the complications listed herein, must be promptly investigated.

ANAPLASTIC LARGE CELL LYMPHOMA
European safety information, the FDA and current scientific literature have identified a possible association between breast implants and the rare development of anaplastic large cell lymphoma (ALCL), a
type of non-Hodgkin’s lymphoma. Women with breast implants may have a low but higher risk of developing ALCL in an area adjacent to the implant. This specific entity is included in the WHO 2016
classification under the terminology “ALCL – AIM”. These safety reports have advised that an extremely small number of cases have been reported in either the fluid or scar tissue adjacent to the implant.
The main symptoms of ALCL in women with breast implants reported are, persistent swelling or pain, late onset persistent peri-implant seroma, capsular contracture or a mass in the vicinity of the breast
implant.

In 2011 the FDA reported that although there might be an increased risk of developing ALCL in women with breast implants, the numbers are so low that it is difficult to conclude a link between the two with
any certainty. This statement was updated in 2016 with additional information regarding the nature of ALCL and recommendations for testing and treating this rare disease. If women present with any of the
aforementioned symptoms, or if ALCL is suspected, it is advised that patients should be assessed to rule out peri-implant ALCL. When testing for ALCL, collect fresh seroma fluid and representative portions
of the capsule and send for pathology tests to rule out ALCL. Diagnostic evaluation should include cytological evaluation of seroma fluid with Wright Giemsa stained smears and cell block
immunohistochemistry testing for cluster of differentiation (CD) and Anaplastic Lymphoma Kinase (ALK) markers. Most cases of breast implant-associated ALCL are treated by removal of the implant and
the capsule surrounding the implant and some cases have been treated by chemotherapy and radiation. In the case that your patient patient is diagnosed with ALCL, a report should be made to Eurosilicone
via the e-mail address: [email protected] and an individual treatment programme should be initiated by a multidisciplinary team.
INSTRUCTIONS FOR USE
SURGICAL PROCEDURE
Correct surgical procedures and techniques are the responsibility of the medical profession. Each surgeon must evaluate the suitability of the procedure based upon current accepted techniques, indi-
vidual judgement and experience. Proper size and shape of implants must be determined for the individual patient by the surgeon. An incision should be of appropriate length to accommodate the style,
size, and profile of the implant. This will reduce the potential for creating excessive stress to the implant during insertion.

SPECIFIC GUIDELINES FOR THE USE OF EUROSILICONE MAMMARY IMPLANTS
1. Size selection should be decided by base dimensions, the patient’s dimensions, and individual breast characteristics rather than preferred volume or projection.
2. The pre-formed shapes of the Eurosilicone implants are firmer than other cohesive gel filled implants, and the surgical incisions should be longer than those required for compressible implants
3. Where appropriate the new inframmary fold may be required to be approximately 1cm below the natural fold
4. The recommended approach is via the inframammary fold. It is the responsibility of the surgeon to decide if axillary or trans areolar placements may be possible. We caution that insertion and
orientation may be more difficult due to the increased firmness of the gel.
5. Sub-muscular placement (with possible lower ligament release) is preferred where possible. As the gel is highly cohesive it has a more defined radius on the upper apex margin. Sub-muscular
placement reduces the probability of this apex radius being palpable.
6. DO NOT use force during placement of Eurosilicone implants. Over manipulation of Eurosilicone implants may result in permanent distortion of the cohesive gel and loss of the implant’s shape
7. Maintenance of correct alignment and avoidance of rotation in the post-operative phase is aided by:
a) Achieving a surgical pocket closely aligned to the implant.
b) Achieving a “dry” surgical pocket.
c) Practising the procedure of implant stabilisation by use of post-operative external immobilisation.
8. All mammary implants have the potential in the immediate post-operative phase to rotate and the above guidelines will assist in avoidance of this with any anatomically shaped implants.
9. CORRECT ORIENTATION IS VITAL when using any anatomical design of Eurosilicone implant, due to the conical shape of these implants having an apex. Correct orientation during implantation is
aided by orientation markers as below.

SUPPLIED STERILE
Implants are supplied in sterile form (Ethylene Oxide Sterilization), processed by validated strictly controlled sterilisation cycles with ethylene oxide. Sterility is verified in accordance with standards. Sterility
of the implant is maintained only if the package is intact and undamaged. NEVER use an implant with damaged protective packaging.

SINGLE USE
Implants are intended for SINGLE USE ONLY. DO NOT REUSE EXPLANTED PRODUCTS. DO NOT RESTERILIZE ANY PRODUCT. Explanted products should not be reused because recleaning
and resterilization procedures may not adequately remove biological residues, such as blood, tissue and other matter, which could retain resistant pathogens and may also affect the performance of
the implant.

PACKAGING
Sterile product is supplied in a sealed, double primary package. Sterility is not guaranteed if the package has been damaged or opened. Tear-Off patient record labels are attached to the primary package.
These labels are to be attached to the patients’ records.

STORAGE CONDITIONS
Eurosilicone products must be stored in normal conditions, must be protected from humidity and direct sunlight and must be store with the arrows pointing up.
In these conditions, Eurosilicone products have a shelf life of five years.

TO OPEN PACKAGED STERILE PRODUCT
1. Peel open outer package under clean, aseptic conditions, over sterile field, allowing sealed inner package to fall gently into the field.
2. Attach patient record portion of the inner label to patients chart.
3. Using aseptic precautions, peel open the inner package.

PRODUCT EXAMINATION AND HANDLING
1. Product should be visually examined for any evidence of particulate contamination, damage or leakage.
2. The implant should be kept submerged in sterile water or normal saline prior to implantation to prevent contact with airborne and surgical field particulate contaminants.
3. Do not immerse the implant in Betadine or Iodine containing solutions. If Betadine or Iodine containing solutions are used in the pocket, ensure that it is rinsed thoroughly so no
residual solution remains in the pocket
4. Gel-filled mammary implants may contain air bubbles. This occurs normally during sterilisation and has no effect on product integrity or performance.

CONTAMINATION OF IMPLANTS
Care must be taken to prevent surface contaminants such as talc, dust and skin oils from coming into contact with the implant. Products should be inspected for contamination prior to insertion. Con-
tamination at the time of surgery increases the risk of periprosthetic infection, and possibly capsular contracture. Textured silicone surfaces are potentially more susceptible to contamination than smooth
silicone surfaces. Extra care should be taken in the handling of textured surface implants. Eurosilicone accepts no liability for products contaminated by other substances after the product leaves our
possession.

SURFACE CONTAMINANTS (TALCS, DUST, LINT, OILS) ON SURFACE OF IMPLANTS CAN CAUSE FOREIGN BODY REACTION, HANDLE WITH CARE WITH SURGICAL GLOVES (RINSED
FREE OF TALC) WITH STRICT ASEPTIC TECHNIQUE. DO NOT IMPLANT CONTAMINATED PRODUCT.
Back-up implants must be readily available at the time of surgery for use in the event of contamination.

DAMAGE OF IMPLANTS - BREAKAGE AT SURGERY
Extreme care should be taken in the use and handling of implants to minimise the potential for breakage of shells. All implants have been produced by established manufacturing techniques and under
strict quality control standards, but there may be some breakage of implants during handling or in surgery, both in initial and any subsequent surgeries. Utmost care must be taken to avoid contact with
sharp objects, such as surgical instruments, suture needles or hypodermic needles. Avoid undue handling with blunt instruments or manipulation. Implants should be carefully inspected for structural
integrity prior to use. Damaged products should not be implanted, do not attempt to repair damaged products. An extra product should be available at the time of surgery in the event of damage to, or
contamination of, the implant. Recommended procedures for testing, examination and handling of products should be meticulously followed to assure proper use of implants. Patients should be instructed to
inform other treating physicians of the presence of implants to minimise the risk of damage.

ACCIDENTAL RUPTURE OF IMPLANTS IN SURGICAL POCKET
If this occurs, the gel mass may be penetrated with index finger of double gloved hand, whilst exerting pressure on the breast with other hand. The gel can be manipulated out of the cavity, within the hand, and
the outer glove can then be pulled over the mass for disposal. Blot the surgical pocket with gauze sponges. Isopropyl alcohol will aid the removal of gel from instrumentation.

ALTERATION OF IMPLANTS
No alteration should be made to the mammary implant before implantation. Alteration of implants voids all warranties, express or implied.

IMPLANTS ELIMINATION
All mammary implants must be eliminated according to requirements related to medical wastes involving an infectious risk.

RETURN OF GOODS POLICY
Prior to the return of any product:
a) Product File Notes must be filled.
b) Decontamination certificate must be supplied with any product returned which has been removed from the packaging.
c) If it is necessary to puncture the shell of any sealed shell product to assist with safe decontamination sterilisation procedures, the area of mechanical interference should be marked with indelible
marker on the surface of the product and reference made on the decontamination certificate.
d) Products File Notes and returned products must be sent to Eurosilicone S.A.S. by your distributor only.

WARRANTY
The manufacturer warrants that reasonable care was used in the manufacture of these products, and will replace any product that the manufacturer’s investigation shows to be defective at time of ship-
ment. Patient selection, surgical procedures, post-surgical treatment and stresses, and handling of the devices are totally the responsibility of the customer. Eurosilicone has no control over the conditions of
use and can not warrant good effect or against ill effect following the use of the device, and shall not be responsible for any incidental or consequential loss or damage or expenses directly or indirectly
arising from the use of this device. All other warranties (whether implied by law or otherwise) are excluded to the fullest extent permitted by law.

CAUTION:
Federal (USA) law restricts this device to sale by or on the order of a physician.

INDICATIONS
Les implants mammaires Eurosilicone remplis de gel sont indiqués dans les cas suivants:
• La chirurgie esthétique d’augmentation mammaire
• L’augmentation et/ou la correction du contour d’anomalies congénitales mammaires
• La reconstruction suite à une mastectomie sous-cutanée ou d’autres procédures relatives à la mastectomie ou traumatismes
• La combinaison d’anomalies mammaires et thoraciques
• Le remplacement d’implants pour des raisons médicales ou esthétiques

CONTRE-INDICATIONS
L’utilisation de ces implants est contre-indiquée chez les patients présentant une ou plusieurs des conditions suivantes:
• Maladie FIBROCYSTIQUE
• Couverture TISSULAIRE INSUFFISANTE liée à des dommages consécutifs d’irradiation du torse, des greffes thoraciques de peau très serrées, ou une résection radicale du muscle grand pectoral
• Présence d’un CARCINOME MÉTASTATIQUE ou LOCAL de la poitrine
• Système immunitaire affaibli/compromis
• Historique de sensibilité aux matériels étrangers ou atopie ou HISTORIQUE NON SATISFAISANT d’augmentation/reconstruction
• Patient PHYSIOLOGIQUEMENT/PSYCHOLOGIQUEMENT inapte
• INFECTION en cours d’une partie du corps
• Historique récent d’abcès des seins
• Historique de difficulté de cicatrisation ou gonflement local
• Toute autre condition médicale sérieuse
• Ces dispositifs ne sont pas prévus pour le traitement d'enfants ou de femmes enceintes ou allaitantes

IMPORTANT : Il incombe au chirurgien d’effectuer les évaluations médicales relatives à l’éligibilité du patient à l’implantation, de décider de la technique chirurgicale la plus appropriée à la fois au patient, et
au type d’implant choisi.
L’implantation mammaire est connue pour apporter une satisfaction psychologique chez les patients. Cependant comme toute autre procédure médicale des risques et des complications peuvent
survenir. Préalablement à l’implantation, le chirurgien doit mettre à disposition du patient toutes les informations contenues dans ce livret d’instruction d’utilisation, s’assurer qu’il en a compris tous les
termes et recueillir son consentement éclairé.
Le chirurgien se doit d’informer le patient du rapport bénéfices/risques associé à cette intervention. Chaque complication potentielle et avertissement doit être discuté avant la chirurgie. Le patient doit être
informé du fait que les implants ont une durée de vie limitée et que la pose d’un implant est susceptible de nécessiter un retrait ou un remplacement, ce qui peut impliquer une nouvelle intervention chirurgicale.
Le suivi clinique des implants Eurosilicone permet, à ce jour, d’établir une durabilité de l’implant de 82.4%, après 10 ans d’implantation (risque de survenue de la première ré opération= 17.6%, méthode
Kaplan Meier).
Lors d’une mammographie, le radiologue devra être informé de la présence de l’implant afin de pouvoir adapter la compression mammographique.
Pour toute intervention chirurgicale au niveau des seins, le chirurgien ou le médecin devra être averti de la présence de l’implant.
Avant toute application de médicaments topiques (comme les stéroïdes) au niveau des seins il est nécessaire de consulter un médecin ou un pharmacien.
La présence d’implants mammaire peut potentiellement retarder la détection d’un éventuel cancer du sein par auto examen. C’est pourquoi, le patient doit être informé de la nécessité de consulter un
chirurgien et/ou un médecin pour réaliser le suivi médical adéquat, ainsi qu’un dépistage régulier du cancer du sein. Le patient doit également être informé de la nécessité de consulter un médecin pour
toute suspicion de complication notamment en cas de traumatisme ou de compression causée, par exemple, par un massage violent des seins, une activité sportive ou l’utilisation de ceintures de sécurité.
A l’issue de la chirurgie, le praticien remettra la carte d’identification du dispositif au patient qui devra la conserver en permanence sur lui afin de, notamment, faciliter les soins médicaux d’urgence.
N.B.: Bien que les implants Eurosilicone n'aient pas été spécifiquement testés en vue d'examens IRM, veuillez noter que les implants mammaires remplis de gel Eurosilicone sont tous fabriqués à partir de
matériaux en silicone à usage médical compatibles avec les examens IRM. Le matériel implantable en silicone de haute qualité est identique à celui utilisé pour les autres implants de silicone Eurosilicone,
avec lesquels certains patients ont passé des examens IRM sans que des problèmes de compatibilité aient été signalés à ce jour.

COMPLICATIONS POTENTIELLES ET AVERTISSEMENTS
Les complications connues associées aux prothèses mammaires pré remplies de gel sont les suivantes:

Les autres complications potentielles associées à toute procédure chirurgicale devront être discutées avec le patient. Ceci inclus sans limitation : l’infection (voir ci-dessous); l’hématome (voir ci-dessous);
l’accumulation de liquide séreux (voir ci-dessous); la perte de sensation (voir ci-dessous); la réaction médicamenteuse (voir ci-dessous); la lésion d’un nerf; l’incompatibilité du patient aux corps étrangers; et
la faible capacité de cicatrisation.

CONTRACTURE CAPSULAIRE
La contracture de la capsule fibreuse est une complication fréquente suite à une chirurgie d’augmentation mammaire. Bien que la formation de tissus fibreux autour de la prothèse soit une réponse
physiologique normale à un corps étranger, toutes les capsules ne se contractent pas. La contraction de tissus autour de l’implant peut occasionner une fermeté, une sensation de gêne ou une douleur
des seins. L’étiologie de la contracture capsulaire n’est pas connue, mais elle dépend de plusieurs facteurs. La contracture se développe en degrés qui peuvent varier, unilatéralement ou bilatéralement, et
apparaitre plusieurs semaines ou années après l’opération. Les cas sévères nécessiteront une intervention chirurgicale. La contracture capsulaire peut apparaitre après une capsulectomie. Les gros- seurs
perçues comme des capsules cicatrisantes ont retardées le diagnostic de tumeurs palpables. Les grosseurs suspectes doivent être immédiatement examinées.

CALCIFICATION
La calcification apparait communément dans les tissus mammaires matures avec ou sans implantation. La calcification est aussi connue pour survenir après l’implantation d’un corps étranger, bien
que son étiologie soit inconnue, et que les cas rapportés soient rares. La micro-calcification après une implantation apparait typiquement sur ou autour de la capsule fibreuse en plaques minces ou en
accumulation. Des micro calcifications trop importantes peuvent causer dureté et sensation de gêne au niveau des seins, et peuvent nécessiter une intervention chirurgicale.

CAPSULOTOMIE EXTERNE
Le traitement de la contracture capsulaire par compression manuelle externe peut causer l’affaiblissement ou la rupture de l’enveloppe. Des cas de rupture ont été rapportés, et Eurosilicone ne recommande

pas la procédure par capsulotomie externe, et n’est en aucun cas responsable de l’intégrité de l’implant si des techniques de compression manuelle ou d’autres techniques de stress externe étaient utilisées.

RUPTURE
La rupture du gel peut apparaitre en phase post opératoire suite à une lésion de l’implant pendant la manipulation ou la chirurgie. La rupture de l’enveloppe peut provenir d’une contracture, d’un trauma-
tisme ou d’une manipulation excessive. Des ruptures d’étiologie inconnue ont été rapportées. En raison de la cohésivité du gel, quelques ruptures du gel peuvent rester indétectées jusqu’à la réalisation
d’une chirurgie pour autre raison (par exemple changement de taille). En dépit des propriétés cohésives du gel, l’extravasation hors de la loge chirurgicale peut survenir sous pression. Dans ce cas une
nouvelle chirurgie est nécessaire pour éliminer le gel. L’inflammation et la formation de granulomes de silicone ont été rapportées. Si une rupture de l’enveloppe est suspectée, l’implant doit être retiré.

INFECTION
Une infection préexistante non résolue avant la mise en place de l’implant augmente le risque d’infection péri prothétique. Ne pas exposer l’implant aux contaminants cela augmente le risque d’infection.
L’infection est un risque inhérent à toute chirurgie invasive. L’infection autour de l’implant mammaire peut apparaître dans les jours, semaines et même années qui suivent la chirurgie. Les signes d’une
infection aiguë associée aux prothèses mammaires incluent: Érythème, fragilité, accumulation de fluides, douleur et fièvre. Les signes d’une infection subclinique peuvent être difficiles à détecter. Les
infections postopératoires doivent être traitées agressivement en fonction des pratiques médicales normalisées pour éviter des complications plus graves. Les infections résistantes aux traitements ou
les infections nécrosantes peuvent nécessiter l’explantation de l’implant. La contracture capsulaire peut être liée à une infection dans la zone périprothétique.

GRANULOME
La formation de granulomes est une réponse tissulaire commune à la présence de substances étrangères. Il se peut que la réaction tissulaire soit provoquée par la présence de l’implant, ce qui entraîne la
formation d’un granulome de silicone.

HÉMATOME/SEROME
L’hématome et les accumulations de liquide sont des complications associées à tout type de chirurgie invasive. L’hématome et le sérome post opératoires peuvent favoriser les infections et/ou les con-
tractures capsulaires. L’hématome et le sérome post opératoires peuvent être minimisés par une attention méticuleuse à l’hémostase, pendant la chirurgie ainsi que par l’utilisation post opératoire d’un
système de drainage fermé. Le saignement persistant et excessif doit être maîtrisé avant l’implantation du dispositif. Toute évacuation post opératoire de l’hématome ou du sérome doit être soigneuse-
ment effectuée pour éviter la contamination et les dommages à l’implant.

EXTRUSION
Une couverture tissulaire instable ou inadaptée et/ou une mauvaise cicatrisation peuvent entraîner l’exposition de l’implant et son explantation. Les causes ou les facteurs qui y contribuent, peuvent notam-
ment inclure: une infection, une blessure, une nécrose avec ou sans infection, une contracture capsulaire, une capsulotomie externe, un lambeau cutané inadapté, une taille incorrecte de l’implant, un mau-
vais placement de l’implant, une érosion tissulaire associée aux plis de l’implant. Il a été démontré que l’incidence des extrusions augmente quand la prothèse a été implantée dans des zones lésées : les
tissus cicatriciels, les tissus fortement irradiés ou brûlés ou les zones osseuses écrasées, des zones où une réduction chirurgicale sévère a été effectuée; la loge chirurgicale où des stéroïdes sont utilisés.

PLIS ET RIDES
Des tissus sus-jacents minces ou inadéquats, des patients avec peu ou pas de tissus adipeux sous-cutanés, des implants trop gros pour la loge chirurgicale ou la structure anatomique du patient ou la
mise en place sous-cutanée, peuvent contribuer à l’apparition de rides et de plis palpables et/ou visibles. Les plis peuvent provenir de l’amincissement et de l’usure de tissus adjacents, et de l’extrusion de
l’implant. Des signes d’inflammation cutanée, tels que la fragilité et l’érythème, peuvent indiquer l’amincissement et l’érosion. Les cas douteux doivent être rapidement examinés. Les rides et/ou plis
palpables peuvent être confondus avec une tumeur palpable, et les cas douteux doivent être rapidement examinés.

SENSATION
Le risque de dysesthésie temporaire ou permanente existe à la suite de toute procédure chirurgicale invasive. Une technique chirurgicale méticuleuse peut minimiser ce risque mais non l’exclure. Les
dysesthésies aréolaires et -mammelonaires, et moins fréquemment de la zone mammaire en général, ont été rapportées après implantation et peuvent être temporaires ou permanentes. La possibilité
d’altération neurologique augmente avec l’importance de la chirurgie. L’implantation chirurgicale d’une prothèse mammaire peut altérer la capacité à allaiter. Cependant il faut souligner qu’une chirurgie
antérieure de reconstruction mammaire peut être la cause initiale de cette altération.

INTERFÉRENCE AVEC LA MAMMOGRAPHIE
Les techniques de positionnement normalisées ont montré des limites significatives lorsqu’elles sont utilisées pour radiographier des seins avec implant. L’implant peut interférer avec la détection précoce
du cancer du sein par mammographie en noircissant les tissus des seins et/ou en compressant les tissus sous jacents ce qui peut “masquer” des lésions douteuses du sein. Les patients doivent être
informés sur la nécessité de demander à des radiologistes expérimentés les techniques et équipements radiologiques les plus récents pour leur mammographie. Ils doivent informer leur radiologiste de la
présence, du type et de la localisation des implants. S’’il s’agit d’implants anatomiques, les radiologistes doivent connaître les marques d’orientation sur l’implant anatomique.

RÉSULTATS INSATISFAISANTS
Les complications relatives à la taille incorrecte, aux cicatrices mal placées, aux cicatrices hypertrophiques ainsi que celles listées ci-dessous sont habituellement liées à la technique chirurgicale. Un choix
minutieux de la taille, la création d’une loge chirurgicale appropriée et adéquate à la taille de la prothèse, et l’utilisation de procédures chirurgicales actuellement reconnues relèvent de la responsabilité
des chirurgiens.

DÉPLACEMENT DE L’IMPLANT
Les implants peuvent se déplacer et créer une sensation de gêne et/ou une déformation du sein.
Les techniques d’implantation difficiles peuvent augmenter la possibilité de déplacement en diminuant la taille de la loge et la précision de mise en place. Le déplacement peut nécessiter une interven-
tion chirurgicale.

ASYMÉTRIE
L’asymétrie peut résulter d’une mise en place initiale incorrecte, d’un déplacement ou d’un échec de correction, d’une asymétrie préexistante ou d’une variation de la taille de l’implant individuel. Une
asymétrie préexistante peut ne pas être entièrement rectifiable. L’asymétrie peut aussi être un symptôme de contracture capsulaire, d’accumulation de liquide, d’infection, de dysplasie post opératoire du
sein, d’une différence unilatérale du développement du muscle, ou d’un dégonflement et pourrait nécessiter des examens plus approfondis.

UTILISATION DE MÉDICAMENTS
Eurosilicone ne peut en aucun cas prévoir, ni garantir, la sécurité de l’introduction intraluménale de médicament, notamment des solutions anesthésiques, antibiotiques ou vitaminées. Si une telle
utilisation est envisagée, le fabricant du médicament concerné doit être consulté.

EXPLANTATION DES PROTHÈSES MAMMAIRES
Bien qu’il n’y ait pas de données précises dans la littérature médicale, il est attendu qu’un grand nombre de prothèses mammaires sera explanté ou remplacé. Ceci n’est pas un évènement rare en ce qui
concerne les procédures de chirurgie plastique communément répétées pour conserver la satisfaction du patient. En raison de la grande diversité des réponses physiques des patients à la chirurgie des
implants mammaires, des différentes techniques chirurgicales et des traitements médicamenteux, ainsi que des complications éventuelles, les patients doivent être informés que les implants ne doivent
pas être considérés comme des implants définitif et que la chirurgie d’explantation peut être suggérée à tout moment. Eurosilicone ne garantit aucune durée d’implantation.

ALLERGIE AU SILICONE
Bien que l’allergie au silicone soit un phénomène extrêmement rare, il ne peut pas être complètement écarté.

ALLAITEMENT
L’implantation peut affecter la production de lait durant l’allaitement, en particulier si une incision péri-aréolaire est réalisée.

DOULEURS
Toute douleur inexplicable, non associée à une des complications listées ci-dessus, doit immédiatement être examinée.

LYMPHOME ANAPLASIQUE À GRANDES CELLULES
Les consignes de sécurité européenne, la FDA et diverses parutions scientifiques, ont établi un lien possible entre les implants mammaires et une rare apparition de lymphome anaplasique à grandes
cellules (LAGC), un type de lymphome non-hodgkinien. Les femmes porteuses d’implants mammaires courent un risque faible, mais supérieur à celui de la population féminine en général, de développer
un LAGC dans une zone proche de l’implant. Cette entité spécifique fait partie de la classification de l’OMS de 2016 sous la terminologie « LAGC-IAM ». Ces rapports de sécurité ont indiqué qu’un très petit
nombre de cas ont été signalés dans le fluide ou le tissu cicatriciel adjacent à l’implant. Chez les femmes porteuses de prothèses mammaires, les principaux symptômes de LAGC signalés sont : une
douleur ou une inflammation persistante, une apparition tardive de liquide autour de l’implant, une contracture capsulaire ou une masse autour de l’implant mammaire.

En 2011, la FDA a indiqué que bien qu’il puisse exister un risque accru de développer un LAGC chez les femmes porteuses de prothèses mammaires, les chiffres sont si bas qu’il est difficile d’établir avec
certitude un lien entre les deux. Cette déclaration a été mise à jour en 2016 grâce à des données complémentaires détaillant la nature du LAGC ainsi que des recommandations pour tester et traiter cette
maladie rare. Si les femmes présentent l’un ou l’autre des symptômes susmentionnés, ou si un cas de LAGC est soupçonné, il est conseillé d’évaluer les patientes pour exclure des cas de LAGC avec
apparition de liquide autour de l’implant. En cas de recherche pour un LAGC, prélever du sérome frais et des parties représentatives de la capsule et effectuer des analyses pour exclure un cas de LAGC.
L’évaluation du diagnostic devra comprendre une évaluation sérologique sous la forme de coloration de frottis Wright-Giemsa et des tests d’immunohistochimie par blocs de cellules pour les clusters de
différenciation (CD) et les marqueurs « Analplastic Lymphoma Kinase » (ALK). La plupart des cas de LAGC associés aux implants mammaires sont traités par explantation et élimination de la capsule
entourant l’implant. Certains cas ont été traités par chimiothérapie et radiothérapie. Dans le cas où le diagnostic de LAGC serait posé, veuillez envoyer un compte-rendu à Eurosilicone via l’adresse e-mail
suivante : [email protected]. Par ailleurs, un programme de traitement personnalisé doit être mis en place par une équipe multidisciplinaire.

INSTRUCTIONS D’UTILISATION

PROCÉDURE CHIRURGICALE
Il incombe à la profession médicale d’utiliser les procédures et techniques chirurgicales appropriées. Chaque chirurgien doit évaluer la conformité de la procédure en fonction des techniques actuelles
reconnues, de son jugement individuel et de son expérience. La taille et la forme appropriée de l’implant doivent être déterminées par le chirurgien pour chaque patient. L’incision doit être d’une longueur
appropriée afin de convenir au modèle, à la taille et au profil de l’implant. Ceci réduira le potentiel de création d’un stress excessif de l’implant durant son insertion.

CONSIGNES SPÉCIFIQUES RELATIVES A L’UTILISATION DES IMPLANTS MAMMAIRES EUROSILICONE
1. La sélection de la taille de l’implant doit être faîte en fonction des mensurations du patient et des caractéristiques des seins plutôt que sur un souhait de volume ou de projection.
2. Les implants anatomiques Eurosilicone sont plus fermes que les implants remplis de gel cohésif, et les incisions chirurgicales doivent être plus longues que celles requises pour les autres
implants remplis de gel cohésif.
3. Quand cela est possible, il peut être nécessaire de positionner le nouveau pli infra mammaire environ 1 cm au dessous du pli naturel.
4. L’approche recommandée est la voie sous-mammaire. Il est de la responsabilité du chirurgien de décider si les voies axillaires ou trans-aréolaires sont réalisables. Nous renforçons notre
avertissement sur le fait que l’insertion et l’orientation peuvent être plus difficiles en raison de l’augmentation de la fermeté du gel.
5. Le placement sous-musculaire (avec une relâche possible du ligament inférieur) est préférable si possible. Comme le gel est très cohésif, il conserve un contour plus défini sur le sommet de la
bordure supérieure. Le placement sous-musculaire réduit la probabilité que ce contour devienne palpable.
6. NE PAS exercer de force durant le positionnement des implants Eurosilicone. Une manipulation excessive des implants Eurosilicone peut engendrer une altération permanente du gel cohésif et
la perte de la forme de l’implant.
7. La rotation des implants lors de la phase post-opérative peut être évitée par:
a) La réalisation d’une loge chirurgicale adaptée à l’implant
b) L’obtention d’une loge chirurgicale “sèche”
c) La réalisation d’une procédure de stabilisation de l’implant en utilisant une immobilisation postopératoire externe.
8. Tous les implants mammaires peuvent potentiellement pivoter lors de la phase postopératoire. Les consignes ci-dessus aideront à éviter ceci avec les implants à forme anatomique.
9. UNE ORIENTATION ADÉQUATE EST IMPERATIVE lors de l’utilisation d’un implant anatomique conçu par Eurosilicone, en raison de la forme conique de ces implants qui possèdent un
sommet. La correction de l’orientation lors de l’implantation est aidée par les repères ci-dessous.

Des marquages d’orientation palpables sont placés sur les faces supérieure et postérieure de l’implant. Se référer au diagramme schématique ci-dessous pour le positionnement des marqueurs.

FOURNI STÉRILE
Les implants sont fournis stériles (stérilisation à l’oxyde d’éthylène). Ils sont traités suivant un cycle de stérilisation validé et contrôlé strictement. La stérilité de l’implant est assurée uniquement si
l’emballage est intact et non endommagé. NE JAMAIS utiliser un produit dont l’emballage assurant la stérilité aurait été endommagé.

UTILISATION UNIQUE
Les implants sont prévus pour un USAGE UNIQUE. NE PAS RÉUTILISER DES PRODUITS EXPLANTÉS. AUCUN PRODUIT NE DOIT ÊTRE RESTÉRILISÉ. Les produits explantés ne doivent pas
être réutilisés car les procédures de re-nettoyage et de re-stérilisation risquent de ne pas complètement éliminer les résidus biologiques, tels que le sang, les tissus et autres substances. Des organismes
pathogènes pourraient subsister et les performances de l’implant pourraient être affectées.
