Summary
The evolution of online monitoring of the patient's physiological parameters in the last
twenty years has contributed to increase the number ofcomplex surgical operations. As a
consequence the role ofthe anaesthetist has become morecomplex and he must deal with
a higher number ofcontrol actions. Feedback control may reduce his work load and conse¬
quently increase the safety ofthe patient. Feedback control in anaesthesia was subject to
manyinvestigationsinthepast, butnoneledtopracticalimplementationintheclinicalrou¬
tine. Therefore, all control tasks ofthe anaesthetist arestill performed manually. The main
questions toanswerduringthisworkweretodetermineifautomaticfeedbackcontrolcanbe
applied with an acceptable performance in routine cases, ifit is possible to apply different
kinds ofcontrol techniques and finally, which controller performes best in this biomedical
field. We considered the feedback control ofinhaled and exhaled gas concentrations, which
are routinely measured online, and ofthe mean arterial pressure (MAP). This variablecan
be influenced by the anaesthetic gas concentration in the human body. The system to be
controllediscomposedofabreathingcircuitandtherelevantpartofthepatient'sbody(rep¬
resented bythepharmacokinetics andpharmacodynamics) and the necessary actuatorsand
sensors. Althoughsomepreviousinvestigationswereavailableinthemathematicalmodeling
ofsome subsystems (especially for the pharmacokinetics), there were many open question
aboutthedynamicalbehaviourofthesystemtobecontrolled.Inaddition,thereisahighin¬
dividualparametervariabilityandalackofmeasurablevariables (especiallyofbodyinternal
values), which must be considered in the modelingand control phase.
The present work describes the results ofdifferent feedback control techniques applied to
anaesthesia. The first part shortly introduces the reader tothe purposes ofanaesthesiaand
describesthe structure and the characteristicsofan anaesthesiacontrol system. In the sec¬
ond part, the mathematical modelingofthe system to be controlledis described in detail.
Thethird partdescribestheimplementedcontrollers. Threedesign techniques are used: one
experience basedtechnique (fuzzylogiccontrol) andtwomodelbasedtechniques (statefeed¬
back control, and model predictive control). A systematic design procedure (specifications,
modeling,design, simulations,pilot studies and standard clinical studies) has been used.
Whilethemodelbasedstrategies willsystematicallyuse quantitative informationinformof
mathematicalmodels,theexperiencebasedmodelusequalitativeinformationmainlycoming
fromexpertsknowledge. Thesedifferentsources oftenlead todifferent implementation time
scales: the experience based controller can normallybe implementedfaster than the model
based one (which needs a long modeling phase). In contrast, the quantitative information

available in mathematical models allows algebraicor numerical optimisation (which is not
possible usingexperiencebased controllers) and efficient pretuning in simulation.
Goodresultswereachievedwithfuzzy logiccontrolalgorithms.Thistechniquehasprovento
be useful, fast and efficient by translatingthemanualcontrol strategiesand experienceinto
anautomaticcontrolalgorithm.Especiallyefficientwastherealizationofdecisionbasedfunc¬
tionalities. However, with fuzzy logic control the tuning phase was very long and inefficient
ifa substantial improvement ofthe control performance was required. The computational
effort offuzzy logic controllers depends on the implementation type: low computer power
buthighmemoryrequirements areneededifthenonlinearfunctionderivedfromafuzzyrule
engine is memorized as astaticinput outputtable; vice versahighcomputer powerand low
memory are required ifthe rule engine isevaluated in real time.
This work showed that, with a considerable model effort, it is possible to efficiently use a
linearmodel basedcontrol algorithm even for the variabilityofpatients and thecomplexity
ofthesystem. Fortechnicalreasons,thecomparisonofthetwomodelbasedtechniquescould
notbedonewithidenticalfunctionalspecifications (nooutputconstraintswereimplemented
forMPC). Therefore,additionalworkisnecessarytomakedefinitiveassertions.Nevertheless,
thesemethodshaveshowntoproducesimilarresults. The maindifferencescanbelocatedin
thedesign,whichisrathersimplerforMPC andonthe computationaleffortwhichisclearly
lowerforstatefeedback control. MPC implicitlycovers allparticularfunctionalities (such as
input and output constrains, tracking ofparticular trajectories, time dependent weightings
in the performanceindex, etc.), while state feedback control needs additional design efforts
for each ofthese functionalities.
Thesefirstresultshaveshownthatautomaticcontrolinanaesthesiaunderroutineconditions
isfeasible.
Zusammenfassung
Wahrend den letzten 20 Jahren hat die Entwicklung der Mess- und Anzeigegerate phys-
iologischer Parameter von Patienten einen erheblichen Beitrag zur Zunahme der Anzahl
komplexerchirurgisclierEingriffegeleistet. DieAufgabe derAnasthesiearzte wurdedadurch
laufendanspruchsvollerundbeinhaltetheuteeinegrosseZahlRegelaufgaben.ZurEntlastung
desAnasthesistenundzurErhohungderSicherheitwurdenvieleVersucheunternommen,um
dieseAufgabenzu automatisieren. DakeiuedieserUntersuchungenzueinerklinischenAn-
wendungfiihrte,werdenweiterhinalleRegelaufgabenvomAnasthesistmanuelldurchgefiihrt.
Diese Arbeit ist ein Beitragzur Untersuchung der Moglichkeiten,dieseRegelaufgabenweit-
gehendzuautomatisierenundinderKlinikanzuwenden. FolgendeFragestellungenwarenzu
beantworten: Konnen bekannte Regelungsalgorithmen in diesem Anwendungsgebiet erfolg-
reicheingesetztwerden?Wennja,welcheregelungstechnischeVerfahrenkonnenangewendet
werden und mit welchem Erfolg?
IndervorliegendenArbeitbetrachtetenwirdieRegelungvonein-undausgeatmetenGaskon-
zentrationen, diein derKlinikroutinemassiginEchtzeit gemessen werden unddieRegelung
desmittlerenarteriellenBlutdruckes(MAP).DieseGrossewildvonderAnasthesiegaskonzen-
trationimmenschlichenKorperbeeinflusst. DaszuregelndeSystembestehtauseinemBeat-
mungskreis, ausdemKorperdesPatienten (dasdurchpharmakokinetischeund-dynamische
Gleichungen beschrieben wird) sowie aus den notigen Aktuatoren und Sensoren. Obwohl
einigeAnsatzefiirgewisseTeilsystemevorhandenwaren(hauptsachlichzurPharmakokinetik),
bestanden noch viele offene Fragen iiber das dynamische Verhalten des zu regelnden Sys¬
tems. Zusatzlich musste in der Modellierung-und im Reglerentwurfverfahren beriicksichtigt
werden, dasseinehoheindividuelleParameterVariabilitatundeinesehrbeschrankteAnzahl
von messbaren Variablenwertevorhanden waren.
Diese Arbeit beschreibt die Ergebnisse aus dem Einsatz verschiedener Regelungsverfahren
angewendet in dem Gebiet der Anasthesie. Im ersten Teil wird der Leser ins Thema der
Anasthesie und deren Regelung kurz eingefiihrt. Im zweiten Teil wird die mathematische
Modellierung des zu regelnden Systems detailliert beschrieben. Der dritte Teil beschreibt
die implementierten Regleralgorithmen. Drei verschiedene Verfahren wurden angewendet:
ein erfahrungsbasiertes (Fuzzy Logic Regelung) und zwei modellbasierte(Zustandsregelung
und model predictive control (MPC)). Dabei wurde ein systematisches Entwurfsverfahren
angewendet (Spezifikation, Modellierung, Reglerentwurf, Simulation, Test in klinischen Pi-
lotstudien und in Standardstudien).

Wahrend modellbasierte Verfahren systematisch quantitative Informationen in Form von
mathematischen Modellen verwenden, verwendet der erfahrungsbasierte Regler qualitative
Informationen, diehauptsachlichvom Expertenwissen stammen. DieseunterschiedlichenIn-
formationsquellenfuhrenoftzuunterschiedlichen Realisierungszeiten:dererfahrungsbasierte
Reglerkannim Normalfallschneller iealisiertweiden alsdermodellbasierte.Dieses Verhalt-
nis ist mit der langen Modellierungszeit zu erklaren. Dagegen ermoglichen mathematisch
beschriebenequantitativeInformationennumetischeoderalgebraischeOptiiiiierungsverfahreii
sowie effiziente Parametervoreinstellungen in der Simulation.
Gute Ergebnisse wuiden mit Fuzzy Logic Regleralgorithmen eneicht. Dieses Verfahren er-
wies sich als niitzlich, schnell und efficient bei der praktischen Auslegung der verwende-
ten Reglerstrategien der Experten. Vor allem vorteilhaft erwies sich die Realisierung von
entscheidungsbasierten Funktionsblocken. Dennoch erwies sich die Phase der Parametere-
instellungals langwierigund ineffizient vor allem dann, wenn eine deutliche Steigerungder
Regelgiiteverlangtwar. Dei rechnerische AufwanddesYerfahrenshangtstarkvonderImple-
mentierungsart ab: tiefe Rechengeschwindigkeit und hohe Speicherplatzanforderung werden
verlangtimFalle,wo die Fuzzy Inferenz alsstatische Ein- Ausgangstabellegespeicheit wird.
Umgekehrt wird eine hohe Rechengeschwindigkeit und einekleine Speicherplatzanforderung
verlangt, falls in Echtzeit die Fuzzy Inferenz berechnet wird.
DieseArbeit zeigt,dasses mit einem hohen Modellierungsaufwandmoglichist, lineaiemod¬
ellbasierteRegelungsveifahren anzuweden, trotz dei grossen VanabilitatderPatienten und
der komplexen Eigenschaften des Systems. Einen direkten Vergleich zwischen den zwei mo-
dellbasierten Entwurfsverfahren konnte aus technischen Griinden jedoch nicht mit identis-
chen Spezifikationendurchgefiihrt werden (eskonnten keineAusgangsbegrenzurigen in MPC
definiertwerden).WeitereUntersuchungen waiendeshalbnotwendig, umabschliessendeVer-
gleicheduchfiihrenzukonnen. DennochhabenbeideVerfahrenahnliche Ergebnisse geliefert.
Die Hauptunterschiede liegen indem Entwurfverfahren, das ehereinfacherscheint bei MPC
undinderRechenleistung, diedeutlichtieferistbeidem Zustandsregler.MPCdeckt imVer¬
fahrenimplizitviele besondere Funktionalitaten(wie z.B. Ein- und Ausgangsbegrenzungen,
VerfolgungbekannterSollwertprofile,zeitabhangigeGewichtungeninderZielfunktion,usw.),
wahrend ein Zustandsreglerfurjede solche Funktion Zusatzaufwand im Entwurfverlangt.
DieseerstenErgebnissehabengezeigt,dassautomatisierteRegelunginderAnasthesieunter
routinemassigenBedingungen realisierbarist.
