Göğüs Cerahi Anabilim Dalı PARAPNÖMONİK AMPİYEM Tedavinin Temel İlkeleri • Enfeksiyon kontrolü • Sepsis kontrolü • Plevra boşluğundan püy boşaltma • Ampiyem boşluğunu ortadan kaldırma • Hasta cerrah tarafından ilk görüldüğünde genellikle antibiyotik tedavisi altında – Kültürler çoğunlukla negatif • Pnömoninin hastane ya da toplum kaynaklı oluşuna göre antibiyotik Torasentez • Lokal anestezi • 18 G iğne • Yeri: radyografideki opasitenin 2 cm altı • Torasenteze bağlı pnömotoraksı önlemek için manotmetre tekniği kullanılır – Plevra sıvı basıncı 0 veya negatifse pnömotoraks ve şok olabilir • Manometre tekniği • Anjiokatetere bağlı ince bir tüp ile plevra basıncı saptanır • İnce tüp manometre görevi görür • Sıvı basıncı pozitif ise sıvı rahatça alınabilir Torasentezle tedavi şartları • Benign görünümlü sıvı • Lokülasyonsuz sıvı • Alttaki akciğerin ekspansiyon yeteneğinin olması • Parapnömonik efüzyonda yararlı • Ampiyemde yinelenen torasentezle başarı %25-40 • Büyük (>1.5 L) sıvı boşaltılması: – Re-ekspansiyon pulmoner ödemi !! • Torasentezler sıvıyı loküle hale getirebilir GÖĞÜS TÜPÜ • Tek mutlak endikasyonu: – Efüzyonda bakteri bulunması • Aşikar ampiyemlerde • pH<7.2; glukoz <40 mg/dl; LDH > 1000 IU • Kültür veya gramda mikroorganizma
