Summary
Acutecardiacarrhythmiasarethemaincauseofdeathinindustrializedcoun-
tries. Despitetremendousclinicalimportance,themechanismsbehindtheonset
anddynamicsofcardiacarrhythmiasarestillpoorlyunderstood. Themostdan-
gerous cardiac arrhythmias are ventricular tachycardia and ventricular fibrilla-
tion. The latter arrhythmia is lethal unless treated within several minutes. The
formerarrhythmiaisverydangerousbecauseitcanspontaneouslyconvertinto
thelatter,which,inturnbecomeslethalifleftuntreatedforseveralminutes.
The heart is an excitable medium where waves of excitation can propagate.
These waves coordinate the synchronous contraction of cardiac muscle, mak-
ingthecardiacmuscleworksynchronously. Cardiacarrhythmiasareassociated
with formation of reentry activity and spiral waves. Once formed, the spiral
wavesactaspacemakerswhichexcitethetissuewithhighfrequency. Therefore,
thespiralwavesareoftenreferredtoasthearrhythmiasources. Tounderstand
the formation and the dynamics of the spiral wave is one of the main research
questionsofthefield.
Normally,acardiacarrhythmiaisanexceptionalevent,sincetheheartisca-
pableofdoingabout2billioncontractionswithouttriggeringrhythmabnormal-
ities. Therefore, cardiac rhythms disruptions are associated with pathophysio-
logicalconditions. Suchconditionsareoftenamplifiedinthepresenceofcardiac
arrhythmias,resultinginpositivefeedback.
Recentadvancesincomputationalsciencesandtheamountofdataaccumu-
latedbyinvitroandinvivoexperimentsallowsimulatingthebehaviorofcardiac
rhythmswithmathematicalmodeling. Thisapproachcomplementsexperimen-
talstudiesandclinicalinvestigationsbyallowingtheperformanceofwiderange
ofnumericalexperimentsthatcanpinpointtoaparticularmechanismresponsi-
bleforagivenphenomena.
Inthisthesisweusemathematicalmodelingandnumericalmethodstostudy
theeffectsofpathophysiologicalconditionsonarrhythmogenesis. Ourfocuslies
ontwopathophysiologicalconditions:cardiacischemiaandmyocardialfibrosis.
Cardiacischemiaisalackofaproperbloodsupplyintheheart,whichoftentrig-
gersventricularfibrillation. Conversely,onceventricularfibriallationispresent,
itisalwaysaccompaniedbycardiacischemia.Myocardialfibrosisisastatechar-

acterizedbyanexcessofconnectivetissueintheheart,whichmayhaveadrastic
effectoncardiacexcitationdynamics.
Weshowthatglobalcardiacischemiastabilizesthecardiacactivationpattern,
andreducesthechanceforfibrillationtobespontaneouslyterminated. Wechar-
acterizetheeffectofeachischemiafactorontheactivationpatternandfindthe
factors responsible for acceleration and deceleration of activation. Using these
resultstogetherwiththeresultsofaclinicalinvestigation,weestimatethetime
dynamicsofischemiaprocessesforthefirstfewminutesofischemiainthehu-
manheart.
Next, we study the effect of the spatial distribution of fibrosis on the onset
ofcardiacarrhythmias. Wedemonstratethatatissuewithhomogeneousdistri-
bution of fibrosis is less susceptible to arrhythmia induction than a tissue with
heterogeneousdistributionoffibrosis. Ourstudyshowshowthedynamicsand
the mechanism of induced activation patterns depend on the fibrosis distribu-
tion. Wefindthattheperiodofactivationpatternsisdeterminedbytheregions
withtheslowestarrhythmiasources. Wealsodiscoverthemechanismoftheun-
derlyingarrhythmiaandprovideinsightsabouttheablationprotocolsthatcould
terminatetheactivity.
Finally, we investigate the reason why the period of the activation pattern
isdeterminedbytheregionswiththelowestfrequencyofspiralrotationinthe
case of a heterogeneous fibrosis distribution. We show that such regions can
attractthearrhythmiasources. Weinvestigatethemechanismforthisattraction
andprovidetheconditionswhenthisattractionispossible. Wecharacterizethis
effectintermsofECGmorphologyandshowthatthisattractionisalsoobserved
inclinicalstudies.
In addition, we develop a patient specific model of the geometry of the left
humanventricle. ThismodelisbasedontheclinicalMRIdata
Inaddition,wedevelopapatientspecificmodelofcardiacmyofiberorienta-
tiontostudytheeffectofattractionthemagneticresonanceimagesofpatients.
Weconcludethatmathematicalmodelingisaninvaluablesourceofinsights
into the mechanisms behind cardiac arrhytmias. Our research shed new light
ontheoveralldynamicsofspiralwavesinarrhythmiasandthemechanismsun-
derlyingtriggeringofarrhythmicsources,subjecttovariouspathologicalcondi-
tions. We believe that our results are useful for development of more effective
ablationprotocolsfortreatingarrhythmiasandformakingpredictionsonhowa
particulartissuearchitectureissusceptibletoarrhythmiainductionintheheart.

Samenvatting
Acute hartritmestoornissen zijn de primaire doodszoorzaak in ge¨ındustria-
liseerde landen. Ondanks de overweldigende klinische relevantie, worden de
mechanismen die zich schuilhouden achter het ontstaan en de dynamica van
hartritmestoornissen nog steeds zeer slecht begrepen. De gevaarlijkste hartrit-
mestoornissenzijnventriculairetachycardieenventrikelfibrilleren. Delaatstge-
noemdeisdodelijktenzijhetbehandeldwordtbinnenenkeleminuten,terwijlde
eerstgenoemde zeer gevaarlijk is aangezien het spontaan in ventrikelfibrilleren
kanevolueren.
Hethartiseenexciteerbaarmediumwaarinexcitatiegolvenzichvoortkun-
nenbewegen. Dezegolvencoo¨rdinerendecontractievanhethartopdergelijke
wijzedatdehartspiersynchroonwerkt. Hartritmestoornissenwordengeassoci-
eerdmethetontstaanvan‘reentryactivity’enspiraalgolven.Eenmaalgevormd,
spelen de spiraalgolven de rol van pacemakers die het weefsel exciteren met
hogefrequentie. Daaromwordenspiraalgolvendikwijlsaangeduidalsdebron
vanderitmestoornissen. Detotstandkomingendynamicavandespiraalgolfis
eenvandebelangrijkeopenonderzoeksvrageninhetveld.
Eigenlijkiseenhartritmestoorniseenbuitengewoonfenomeenaangezienhet
hartinstaatisomongeveer2miljardkeersamentetrekkenzonderafwijkingen
inhetritmeteveroorzaken.Omwillevandezeredenwordenhartritmestoornis-
sengeassocieerdmetpathofysiologischeomstandigheden. Zulkeomstandighe-
den worden dikwijls versterkt door de aanwezigheid van hartritmestoornissen
aangeziendezeeenpositievefeedbackgeven.
Recentevooruitgangindecomputerwetenschappenendehoeveelheiddata
dieverzameldwordtininvitroeninvivoexperimentenlatentoeomhartritmes
te simuleren met behulp van wiskundige modellering. Deze aanpak is com-
plementair met experimentele studies en klinische onderzoeken aangezien het
toelaatomeengrotehoeveelheidnumeriekeexperimentenuittevoeren. Hierbij
kan gefocust worden op een specifiek mechanisme verantwoordelijk voor een
gegevenfenomeen.
In deze thesis gebruiken we wiskundige modellering en numerieke metho-
den om de effecten van pathofysiologische condities te bestuderen op het on-
staanvanhartritmestoornissen. Onzefocusligtbijtweepathofysiologischecon-

dities: cardiale ischemie en fibrose van het myocardium. Cardiale ischemie is
eengebrekaangoedebloedtoevoerinhethart,watdikwijlsaanleidinggeefttot
ventrikelfibrilleren. Eenmaal ventrikelfibrilleren aanwezig is in het hart, is er
bovendien ook steeds ischemie te vinden. Fibrose van het myocardium is een
toestandgekarakteriseerddooreenteveelaanbindweefselinhethart,watdras-
tischkanwijzigendedynamicavandehartexcitatie.
We tonen aan dat globale ischemie van het hart het activatiepatroon stabili-
seertendekansreduceertdatfibrillatiespontaaneindigt. Wekarakteriserenhet
effect van elke ischemiefactor op het activatiepatroon en vinden zo de factoren
verantwoordelijk voor het versnellen en vertragen van de activatie. Wanneer
deze resultaten gebruikt worden samen met de resultaten van klinische onder-
zoeken,schattenwedetijdsdynamicavanischemieprocessenafvoordeeerste
paarminutenvanischemieinhetmenselijkhart.
Vervolgensbestuderenweheteffectvanderuimtelijkeverdelingvanfibrose
op het ontstaan van hartritmestoornissen. We tonen aan dat weefsel met een
homogeenverdeeldedistributievanfibrosemindervatbaarisvoorinductievan
ritmestoornisdanweefselmeteenheterogeenverdeeldedistributie.Onzestudie
toontaanhoededynamicaenhetmechanismevange¨ınduceerdeactivatiepatro-
nen afhangen van de distributie van fibrose. We vinden dat de periode van de
activatiepatronenbepaaldwordtdoorderegio’smetdetraagsteritmestoornis-
bronnen.Webeschrijvenookhetmechanismevanhetactivatiepatroonenbieden
inzichtoverablatieprotocolsdiedeactiviteitkunnenstoppen.
Tot slot onderzoeken we de reden waarom de periode van het activatiepa-
troon bepaald wordt door de regio’s met de laagste frequenties voor spiraalro-
tatie in het geval van een heterogene distributie van fibrose. We tonen aan dat
zulke regio’s de bronnen van ritmestoornissen kunnen aantrekken. We onder-
zoeken het mechanisme voor deze aantrekking en bepalen onder welke voor-
waarden deze aantrekking mogelijk is. We beschrijven dit effect in termen van
de morfologie van het electro-cardiogram (ECG) en tonen tevens aan dat deze
aantrekkingookgeobserveerdwordtinklinischestudies.
Daarnaastontwikkelenweeenpatie¨ntspecifiekmodelvandeorie¨ntatievan
spiervezelsinhethartomheteffectvanaantrekkingtebestuderenopMRIbeel-
denvanpatie¨nten.
We concluderen dat wiskundige modellering een onschatbare bron van in-
zichtbiedtindemechanismenachterhartritmestoornissen. Onsonderzoeklaat
nieuw licht schijnen op de mechanismen van het opwekken en de algemene
dynamicavanritmestoornissenonderverschillendepathofysiologischeomstan-
digheden. Wegelovendatonzeresultatennuttigzijnvoorhetontwikkelenvan
meer effectieve ablatieprotocols bij het behandelen van ritmestoornissen en bij
het maken van voorspellingen voor hoe een bepaalde weefselarchitectuur vat-
baarisvoordeinductievaneenhartritmestoornis.
