Banned Abbreviations
The use of certain abbreviations can be confusing and lead to medication errors. The following abbreviations are
unacceptable and must be replaced by their alternative.

IU international units
QD or QOD daily or every other day
TIW 3 times weekly, or three times weekly
g for microgram mcg for microgram
trailing zero, e.g. 2.0 (confused with 20 mg) 2 milligram
leading decimal, e.g. .2 (confused with 2 mg) 0.2 milligram
