MOC 1: SNP POPULATION

MOC 1.A DESCRIPTION OF OVERALL SNP POPULATION 
MOC 1.A.1 Describe how the health plan staff will determine, verify and track eligibility of 
SNP beneficiaries. 

Health Net (HN) follows the Centers for Medicare & Medicaid Services (CMS) requirements 
contained in Chapter 2 of the Medicare Managed Care Manual (MMCM) and California State 
Medicaid (Medi‐Cal) requirements in the applicable regulations in reviewing each enrollment 
election to ensure that the enrollee meets Special Needs Plans (SNPs) eligibility requirements as 
applicable, prior to submitting the enrollment to CMS for approval. Upon receipt of the 
application, HN verifies the enrollee’s eligibility for the Amber I and Amber II dual eligible SNP, 
based on the plan selected, through one of the following methods:  

Acceptable proof of Medicaid eligibility can be a current Medicaid card, a letter from the 
applicable state agency that confirms the Beneficiary’s entitlement to medical assistance under 
a State plan under Title XIX (Medicaid), or verification through a systems query to a State 
eligibility data system. HN will retain a copy of the acceptable proof in the Beneficiary’s file and 
follow. The following steps will be taken to determine, verify and track eligibility: 

1. Verify Beneficiary’s current Medicaid status based on the plan selected, via the State 
Medicaid eligibility website. 
a. Once this information is verified, HN will document the appropriate HN system then continue 
with the eligibility process outlined in the Processing the Enrollment policy.  
b. If HN is unable to verify dual eligibility the beneficiary will be contacted. If HN is unable to 
reach the Beneficiary, it will be documented in the appropriate HN system with all outreach 
attempts and outcomes.   

2. HN will pend the application if unable to obtain the appropriate proof of Medicaid eligibility, 
per the Processing the Enrollment Request policy. 
a. HN will send the necessary letter within the required timeframes as stated in the Processing 
Enrollment Request policy. 

3. To ensure the verification of dual eligible Members is at least as often as the State Medicaid 
conducts re‐determination of Medicaid eligibility, HN utilizes a monthly file from DHCS, which

reflects the Medicaid status of the Dual SNP members.   

4. If HN is notified that a Member enrolled in a dual SNP plan no longer qualifies for Medicaid 
status, HN provides a six month deeming period for the Member to provide confirmation of 
continued eligibility for the Dual SNP plan special needs status. The deeming period begins the 
first of the month following the month in which HN received information that the Member no 
longer qualifies for the Dual SNP and ends on the last day of the sixth month following.  

Disenrollment for Loss of Dual Status notification: 

a. An initial letter is sent to the Member or legal representative within ten days of receipt of the 
monthly Dual SNP membership Comparison report from the state indicating those members 
that have lost Dual SNP status.   
b. If no response is received from the Member or legal representative, an interim letter is sent 
on the first business day of the fourth month of the deeming period. 
c. At the end of the six month grace period, if no response is received from the Member or legal 
representative, a final letter is sent within one business day following the last day of the period 
of deemed continued eligibility.  The disenrollment notice to the Member and the transaction 
to CMS must be sent within three business days following the last day of the period of deemed 
continued eligibility; however, in no case may the disenrollment notice to the Member be sent 
after the transaction is submitted to CMS. 
d. HN submits a disenrollment (transaction type code 51)  to CMS within two business days 
following the last day of the period of deemed continued eligibility for an effective date of the 
first of the month following the last day of the period of deemed continued eligibility. If at any 
point during this process the Member provides proof of their dual eligible status the 
disenrollment process is cancelled.   
e. Once CMS has confirmed the disenrollment and notified HN via the Transaction reply report 
(TRR), the applicable HN system is updated and the disenrollment confirmation letter is sent to 
the Member.   

Health Net will not retroactively terminate members due to retroactive Medicaid termination. 

If during any time within the six month deeming period, Health Net confirms the member 
regained his/her Medicaid eligibility, Health Net will send the disenrollment cancellation letter 
(the “Notice Requirements” and the “Letter Identifier” sections within this procedure and/or 
the attached “Letter Matrix” provides contract and PBP information for the appropriate letter) 
to the member within ten calendar days of receipt of confirmation or regained Medicaid 
eligibility. 

Notice Requirements:  
Request for Additional Information: To obtain information to complete the enrollment 
request, HN must contact the individual to request the information within ten calendar days of

receipt of the enrollment request. The request may be written or verbal but in either case the 
request must be made within ten calendar days 
Acknowledgement of Receipt: A notice acknowledging receipt of the completed enrollment 
request and showing the effective date of coverage must be provided to enrollee/member. This 
notice must be sent no later than ten calendar days after receipt of the completed enrollment 
request. 
Enrollment Confirmation or Rejection:  HN must notify the enrollee/member in writing of CMS’ 
acceptance or rejection of the enrollment within ten calendar days of the availability of the 
daily TRR, whichever contains the earliest notification of the acceptance or rejection. 

MOC 1.A.2 Describe the social, cognitive and environmental factors, living conditions and co‐
morbidities associated with the SNP Population. 

Health  Net’s  Dual  Eligible  D‐SNPs  (Amber  I  and  Amber  II)  provide  healthcare  services  for 
residents of nine of California’s 58 counties: Fresno, Kern, Los Angeles, Orange, Riverside, San 
Bernardino,  San  Diego,  San  Francisco  and  Tulare.  Please  refer  to  Table  1.1  below  for  a 
breakdown of Amber I and Amber II services areas. California is a large and populous state with 
several distinct geographic regions. Los Angeles, San Diego and Orange are coastal counties in 
the southern region with multiple large urban centers, and San Diego shares its southern 
border with Mexico. Fresno, Tulare, Kern, San Bernardino and Riverside are contiguous inland 
counties that begin in the central valley region and extend all the way down to Southern 
California. These counties are more rural and agricultural although Kern, San Bernardino and 
Riverside also have large cities such as Bakersfield and San Bernardino. San Bernardino is a very 
large county that extends from the western Los Angeles county border to the eastern border of 
Nevada – 90% of the county is desert. San Francisco and Sacramento are populous counties in 
coastal and central areas of the Northern California region. Sacramento is the state capital.

The  HN  D‐SNPs  will  encompass  low  income  members  living  in  urban  and  rural  areas  of 
California. For example, in the urban area of Los Angeles County, 16.7% of individuals were 
living  below  the  federal  poverty  level  in  2015.1  Due  to  high  housing  costs  and  the

unemployment rate, the estimated number of homeless individuals in Los Angeles County was 
approximately  46,874  in  2016.2  In  addition,  families  in  over  one‐third  of  Orange  County 
neighborhoods, specifically in north and north central areas of the county, faced financial 
instability and 22% of residents lived in poverty as of 2014.3 In rural areas such as Fresno and 
Tulare counties, more than one‐fourth of residents (25.2% and 27.5%, respectively) were living 
below the federal poverty level in 2015, significantly more than the rate for California (15.3%) 
overall.4 5 

Historically, the dual eligible population experiences significantly greater health care needs 
compared to the general population, increasing the complexity of care and coordination for 
these members. A study conducted by CMS in 2014 indicated that among the fee‐for‐service 
(FFS) Medicare‐Medicaid population included in the study, 60% had diagnoses across at least 
three of the physical and mental health categorical condition groups, while 25% had diagnoses 
spanning five or more condition groups. Three quarters of the study group had at least one 
heart  related  condition,  and  41%  had  one  or  more  mental  health  conditions  (excluding 
substance use disorders).6 Dual eligible members with behavioral health disorders and co‐
morbidities are at further risk of serious complications and increased need and utilization of 
health care resources. 

In comparison, the 2015 HN Health Risk Assessment (HRA) data showed that 43% of Amber I 
SNP members and 48% of Amber II SNP members cannot shop for their own food. Also, 42% of 
Amber I members and 50% of Amber II members reported that they cannot cook their own 
food. Transportation is an economical factor that contributes to members being able to get to 
doctor appointments, pick‐up prescriptions, and attend social events. About 35% and 40% of 
Amber I and Amber II SNP members, respectively, depended on friends for transportation. As a 
result, 64% of Amber I members and 66% of Amber II members reported that they are unable 
to participate in social activities on a regular basis. Dual eligible members that cannot afford the 
essentials or perform tasks on their own, such as food shopping and cooking, buying clothes, 
dressing themselves and securing housing, experience a social deficit that can impact their 
physical and mental health.

Health Net SNP‐specific demographic information according to Health Net of California (HNCA) 
Epidemiological  and  Demographical  Analysis  –  Medicare  SNP,  reported  that  the  member 
average age as of December 31, 2015 was 68.2 years, which was slightly lower than the 2014 
average of 69.5 years. However, the SNP population was younger on average than the Non‐SNP 
Medicare population by 4.5 years. The overall D‐SNP population (Amber I and Amber II) was 
comprised of approximately 40% males and 60% females based on the 2015 HN HRA data.  

Self‐reported  ethnicity  information  gathered  from  the  2016  Consumer  Assessment  of 
Healthcare  Providers  and  Systems  (CAHPS®)7  for  HNCA  Medicare,  inclusive  of  the  D‐SNP 
populations, revealed that respondents were 76% white, 16% Asian and 9% Black or African 
American. The response counts for other ethnicities (American Indian or Alaska Native and 
Native Hawaiian or other Pacific Islanders) were suppressed for 2016 reporting year CAHPS 
results. Of the aforementioned ethnicities, 24% of members designated themselves as Hispanic 
or Latino. About 41% reported a high school or less education and 59% had some college 
education or higher. 

Reponses gathered from the 2015 HN HRA revealed that English was the primary language for 
69.2% of Amber I SNP members and 63.3% of Amber II SNP members, followed by Spanish 
(27.8% and 28.9%, respectively), and Other (1.4% and 2.1%, respectively). In order to address 
any health literacy issues among SNP members, Health Net provides culturally acceptable and 
readable  materials  for  all  federally  required  threshold  languages  in  California.  Member 
informing  materials  are  available  in  English  and  Spanish  and  upon  request,  in  additional 
languages. Health Net customer service representatives who speak Spanish or other languages 
are also available and required interpreter and language services are provided. Health Net D‐
SNP HRA data indicated that 94% of Amber I members and 91% of Amber II members can read 
in their own language, although literacy may still be an issue. In 2016, Health Net received the 
Multicultural Health Care Distinction by the National Committee for Quality Assurance (NCQA) 
for the third time since 2012. Health Net has earned the two‐year distinction and remains the 
only health plan in the nation to earn the distinction simultaneously for all three lines of 
business (Commercial, Medicaid, and Medicare).

MOC  1.A.3  Identify  and  describe  the  medical  and  health  conditions  impacting  SNP 
beneficiaries. 

Cardiovascular  Disease  (CVD),  which  includes  heart  disease,  heart  failure,  stroke  and 
hypertension, accounts for one in three deaths in California, making it the leading cause of 
death in the state. The total number of deaths from CVD exceeds the combined number of 
deaths from diabetes, chronic liver disease/cirrhosis, accidents, Alzheimer’s disease, influenza

and pneumonia, and chronic lower respiratory diseases. Data collected on the prevalence of 
cardiovascular diseases in California indicate that high blood pressure is present in one in four 
adults, high cholesterol is present in one in three adults, and nearly one in four adults is obese.8 
Diabetes is a leading cause of Cardiovascular Disease. One in twelve adults (2.3 million) in 
California  is  diagnosed  with  diabetes.  Diabetes  is  the  seventh  leading  cause  of  death  in 
California claiming nearly 8,000 lives annually.9 People with diabetes ages 60 years old or older 
are 2‐3 times more likely to report an inability to walk one‐quarter of a mile, climb stairs, or do 
housework compared to people without diabetes in the same age group.10  

In comparison, the Health Net D‐SNP populations, vary from the general state population and 
demonstrate a greater incidence of chronic disease and co‐morbidities. The following incidence 
of diseases and comorbidities self‐reported by Amber I and Amber II members on the 2015 HN 
HRA is summarized in Table 1.2 below:

An epidemiological and demographic analysis is conducted annually to assess the clinical needs 
of HNCA Medicare SNP members (Amber I, Amber II, and Jade). The SNP population is a subset 
of the Medicare population, and has an additional set of care requirements mandated by CMS. 
Another purpose of this activity is to identify trends in the SNP sub‐categories and age groups.

Top 10 primary medical diagnoses and Top 20 prescription analyses, in combination with 
member  demographic  information,  are  presented  to  assess  the  characteristics  of  this 
population. 

In the Inpatient setting, Septicemia, Acute and Unspecified Renal Failure, Respiratory Failure, 
Other Nervous System Disorders and Nonspecific Chest Pain were common to all age groups, 
both SNP and non‐SNP. Members 65 and above, SNP or otherwise, tended to be hospitalized 
for Cardiac Dysrhythmias, Congestive Heart Failure and Acute Cerebrovascular Disease. SNP 
members 50 and older were admitted for Coronary Atherosclerosis/Other Heart Disease while 
SNP members under 50 were admitted for Essential Hypertension. 

With regards to the differences between the groups, Acute Myocardial Infarction was unique to 
the SNP 65‐and‐older age group, COPD was in the top ten list in the 50‐to‐64 age group but not 
in others, while Mood Disorders and Schizophrenia were more pronounced in SNP members 49 
and younger. Osteoarthritis and Pneumonia were in the Medicare non‐SNP top ten list, but not 
in the SNP top ten list. Unlike previous years, Epilepsy did not make it to the top ten list in 2015 
for younger SNP members. 

The Outpatient setting was where the most similarities were found. Diabetes Mellitus (both 
with and without Complications), Essential Hypertension, Spondylosis, Other Connective Tissue 
Disease and Other Non‐Traumatic Joint Disorders were common among all SNP and non‐SNP 
age groups. Cataracts were more likely to be found in members 65 and older than in younger 
members, regardless of whether they belonged to a SNP sub‐group or not. In both SNP and 
non‐SNP groups, Osteoarthritis and Other Skin Disorders were in the top ten list of members 50 
and older. Other Nervous System Disorders were more common in SNP members under 65 but 
not in non‐SNP members and in members 65 and older. 

In the Emergency setting, Abdominal Pain, Nonspecific Chest Pain, Spondylosis and Other 
Connective Tissue Disease were common among all age groups and among SNP and non‐SNP 
members. Members 65 and older, regardless of whether they were SNP members or not, were 
more likely to seek treatment at Emergency rooms for Urinary Tract Infections and Other 
Injuries and Conditions Due to External Causes. Likewise, members 50 and older, both SNP and 
non‐SNP, were commonly treated at Emergency rooms for Other Lower Respiratory Disease.   

Superficial Injury Contusion and Sprains and Strains were more likely to occur in SNP, rather 
than in non‐SNP, members in an Emergency room setting. Members between 50 and 64, both 
SNP and non‐SNP, seemed to be treated for Other Nervous System Disorders and Other Non‐
traumatic Joint Disorders in the Emergency department more frequently than any other age 
group. Members 49 and younger had a high occurrence of behavioral health conditions in the 
Emergency setting:  Mood Disorders, Schizophrenia and Anxiety Disorders were more common 
causes of visits to the Emergency room for this age group.  

There were slight differences among the Amber I, the Amber II and the Jade SNP plans, 
although there were more similarities than differences.

In the Inpatient setting, the Amber plans had six of the top ten diagnoses in common with the 
Jade plan. Half of these common diagnoses were heart diseases (CHF, Coronary Atherosclerosis 
and Cardiac Dysrhythmias).   

COPD was more common in the Amber II plan than in the Jade plan, while Acute Myocardial 
Infarction, Acute Cerebrovascular Diseases, Diabetes Mellitus with Complications and Essential 
Hypertension were more prominent in the Jade plan than in the Amber II plan. The Inpatient 
totals in the Amber I plan were too small to be significant. 

In the Outpatient setting, eight out of ten diagnosis categories were common to all plans. 
Osteoarthritis was more common in the Amber plans than in the Jade plan. 

It is in the Emergency category where the Amber I and the Amber II were most alike. The top 
ten diagnoses in the Amber II plan were also found in the top ten list of the Amber I plan.  
However, both Amber plans had only eight of top ten diagnoses in common with the Jade plan. 

The Top 10 SNP inpatient and outpatient behavioral health diagnoses fell under the broad 
categories of Schizoaffective Disorders, Major Depressive Disorders and Psychoses. 

From January 1 through December 31, 2015, roughly 37.0% of the SNP population (13,254 / 
35,813) had a mental health diagnosis present in Health Net medical claims and encounter 
data, and of these, 1,928 or 14.5% have claims in MHN databases. Note that these diagnoses 
are oftentimes not the primary diagnoses in the medical claims/encounter data. 

Eighteen of the Top 20 Enhanced Therapeutic Classification (ETC) drug categories in the SNP 
population as a whole are also in the top 20 drugs for the Non‐SNP Medicare population.  
Antipsychotics and Medical Supplies are two medication categories that are in the SNP Top 20 
list but not in the non‐SNP Medicare list. Likewise, Anticoagulants can be found in the non‐SNP 
Medicare top ten list but not in the all‐SNP list.   

In review of the Average Days’ Supply, Non‐SNP Medicare members were more likely to have 
been on the indicated medication for a much longer time period than SNP members, with the 
exception  of  Diabetic  Therapy,  Analgesics,  Prostatic  Hypertrophy  Agents,  Asthma/COPD 
Therapy Agents and Antianxiety Agents, where the medication duration was longer for SNP 
members. However, SNP members without exception have much higher utilization rates than 
non‐SNP Medicare members.  Among the SNP population, medication duration was longest for 
Diabetic Therapy agents and shortest for Medical Supplies & DME. 

The utilization patterns of the SNP 65‐and‐over age group most closely resembles that of the 
Non‐SNP Medicare population since 19 of the top 20 medications are in both lists. The patterns 
start to diverge in the 50‐to‐64 age range, but the most noticeable differences can be found in 
the under‐50 age group.
