This directory is for Los Angeles, Placer, Sacramento, San Diego, and Yolo counties, CA.
This directory is current as of October 2015. Some plan providers may have been added or removed from the
list after this directory was printed. We do not guarantee that each provider is still accepting new members.
To get the most up-to-date information about Health Net plan providers you can visit www.healthnet.com/uc
or call our Member Service Department at 1-800-539-4072. From October 1 through February 14, our office
hours are 8:00 a.m. to 8:00 p.m., 7 days a week, excluding certain holidays. However, after February 14, your

Enrollment in a Health Net Medicare Advantage plan depends on contract renewal.
