Municipality List of Independent Providers of Related Services
(April 17, 2017)

In cases where the New York, City Department of Education cannot provide Related
Services for students identified as being in need of these services, the Municipality List
of Independent Providers of Related Services was developed. The purpose of this
Municipality List is to assist parents in locating New York State licensed or certified
(independent) providers of Related Services who have agreed to provide these services
under the specific guidelines set forth by the New York, City Department of Education.
This Municipality List will be used whenever the New York, City Department of
Education issues a Related Service Authorization, which is described as follows:

Related Service Authorization (RSA) : In cases where the New York, City
Department of Education are unable to provide a Related Service by
school or contract agency staff, an RSA letter is issued to the
parent/guardian, advising them of their right to obtain the Related Service
from an independent provider of these services at public expense.

The Municipality List of Independent Providers of Related Services provides the names,
addresses, telephone numbers and bilingual capacities of individuals and agencies who
have expressed their interest and availability to provide these services, if needed. All
individuals listed have submitted copies of their current New York, State license or
certificate in their respective discipline, which as since been verified by the New York,
City Department of Education. In addition, all individuals listed as bilingual have
provided satisfactory evidence of their bilingual proficiency. Current (unless included in
the Municipality List of DOE Independent Providers of Related Services with certain
restrictions) and former employees (for one year) of the New York, City Department of
Education or any other New York, City agency will not be included in the Municipality
List unless they formally request in writing and receive a waiver from the New York, City
Conflict of Interest Committee.

Parents/guardians are under no obligation to utilize any individual on this Municipality
List; this list should not be construed as a recommendation for any particular individual.
Individuals may be periodically added or deleted as availability changes.
